A 65 years old man came to our office complaining about his smile “disappearance “ due to attrition problems in the years. The request was for a minimal invasive treatment on the upper teeth without the need to do great works on the lower arch and on his implant crowns on teeth 25 26 36 46 for economic reasons.
After an estethic evaluation , a waxing of the upper arch followed by a intraoral mock up was made . The treatment plan consisted in ceramic veneers from teeth 13 to 23 , overlay/veneers on 15 14 24 , overlay on implant crown 25 26 . The lower arch was treated with direct additional composite from 33 to 43 and a ceramic overlay on implant crown 4,6. A slight increase in vertical dimension was also planned.With this treatment a good occlusion with normal overjet /ovebite values and normalization of the spee curve was gained.
The first step was a minimal preparation of the upper arch for the ceramic adhesive restorations .The choice of the material has been lithium disilicate for its better mechanical properties and the severe length increase of the teeth.
The bonding of the adhesive restoration was made in a unique session together with the treatment of the lower arch. All the adhesive steps were made under rubber dam isolation.
Thanks to adhesive dentistry a ideal estethic and functional result has been accomplished in this case with minimal to zero biologic sacrifice of the dental elements and keeping in consideration also the patient ’s need.
