BioHPP material as a Prosthetic solution for implant prosthesis nowadays
Abstract
BioHPP became a rapidly
growing material in the market of Dental prosthesis in implant dentistry
because it solved plenty of implant Direction problems, compromised Gum and
Bone levels, load and it’s the best modality for screw retained prosthesis for
better retrieval & Oral hygiene.
Introduction
In the past we used to use
cement retained implant prosthesis, which didn’t allow us full retrieval and
performing oral hygiene measures, also the prosthesis was heavy constructed of
metallic framework and porcelain or zirconia coverage later, And the biggest
issue is the treating with compromised ridges after surgical resections which
needed long clinical crowns with connections added to the abutment till BioHPP
appearance & solving of all of these problems with more advantages.
BioHPP is a material based on
PEEk (polyetheretherketone), which has already been successfully used in
operations carried out on humans for many years. Thanks to its excellent
stability, its optimal ploishiable properties and its low plaque affinity, which
allow us to produce high-quality prosthetic restorations and the most preferred
feature is modulus of
elasticity of around 4 GPa, BioHPP which is about
as elastic as bone, helping mitigate any stress that might develop
and reduces stress shielding which give us a guarantee for a life
time implant preservation.
So as any material, there
should be a disadvantage, which I think from my expernice, that it can be
summarized in three issues, 1-BioHPP needs a well qualified and trained dental
practitioner because handling of the material has a big impact on the final
outcome, 2-some people think that in many cases we will need to make veneers on
the BioHPP teeth for superior esthetics which I didn’t feel it as a big issue,
finally It maybe needed to be unscrewed, sent to lap again & again till
getting a totally compatible & hygienic prosthesis with gum.
Case Report
A 45-year-old man reported to our department with
the complaint of missing upper right central. The tooth had been extracted due
to mobility, and the left central and lateral were in mobility grade III, so he
wanted to replace them with a fixed prosthesis. After an initial intraoral
examination, treatment options of a fixed partial denture and an
implant-supported prosthesis were discussed with patient; he agreed to the
implant-supported prosthesis as this was more predictable and conservative to
teeth.
The patient was a smoker in stable general
health, and he had diabetes in the medical history. The intraoral examination
showed that he had poor gingival and periodontal health. The residual ridge had
insufficient width mesio-distally and bucco-lingually and was covered by
inflamed and recessed gum. On radiographic examination, ridge needed bone
grafting in the plan. After measuring the bone clinically and radiographically,
it was decided to place a fixture with diameter root form implant (3.5 × 13-15
mm, Dentium Dental Implant System in the two centrals areas & ( 3.2mm * 11)
in the lateral area.
Procedure
-Punching the Gum over the
fixtures then delivering healing abutment for one week.
-GH measure determined which
is the distance from fixture till the Gum margin to determine the suitable
abutment for proper finish line position.
-Impression copings attached
to the fixtures then connected to each other by Duralay acrylic resin
-Then Open impression
technique used with special tray designed with holes above the copings screws
to allow unscrewing after impression material gets hard.
-Butty&light imp with
opposing alginate impression plus wax bite sent to the lab.
-Lab sent me back try in, it’s
about oven cured acryl making the three abutments one unit.
-I have checked for proper seating & occlusion& tooth shade then I asked for final delivery.
Discussion
BioHPP material gave us a wide
space and variety of options in ttt of edentulous spaces and ridges,
It allowed us bring people
into mouth service in short time with definitive, simple steps, but make sure
from the handling of the lab technician because it differs a lot, And the most
thing that helps him is providing him with accurate impression.
Conclusion
Using BioHPP in single crowns
& telescopic removable overdenture is very useful too, its promising
material that will develop more & more to give us more Facilities in implant
prosthetics ttt options.
