Brief pt. history:
•42 years old male.
•Single.
•Non- smoker.
•Medically fit.
•History of failing restorations and multiple extractions.
•Wants to have something fixed, to have nice smile and eat properly.
•Fair OH.
and Tx plan:
•Scaling, OHI.
•Insertion of upper and lower acrylic dentures at increased OVD. (patient had difficulty in wearing lower denture because of the sharp bony exostosis. He needed to have bone osteoplasy in the lower right side to be able to wear the denture).
•Assessment of pt. acceptance of increase of OVD.
•Replacement of old fillings, caries excavation, assessment of restorability of present teeth and core build up if restorable for crowning (including the UR8 which might be a strategic tooth in the future) .
•RCT of UL1.
• Bone graft in the area of UR1, crown lengthening of UL1,2 and extraction of UR3.
•Replacement of upper anterior teeth by implants.
•Replacement of lower posterior teeth by implant supported bridges.
•Replacement of lower anterior teeth by 6 unit bridge (extraction of LR2 to avoid having double abutments).
