This is a case of a full-arch metalloceramic restoration which, I should admit, was the first big treatment that I have ever worked out together with my amazing laboratory Estadent Labor in Sofia, Bulgaria. I had done this 14-element-pontic before I ever did a three-element one. I was 27 years old, working from the very first day all alone in my office and I told the patient I have no experience in this kind of treatment. However, he really insisted that I should take the case and so we started, with so much excitement.
Originally, the patient came after being hit by a close member of the family and he needed a forensic dental certification for the use in the Court (I have this further study in Forensic Dentistry), so this is how we firstly met. He wanted to do something about his teeth. We first did the digital planning of the case using the shape of his own teeth bringing them back to their original position (and very slightly widening them). After that we made the first provisionals. In the preparation visit I prepared the nine teeth that would have to stay in the mouth and extracted 12, 11, 21 and 22. Then we made a rebasation of the provisionals and the patient went out of the dental office with his future smile.
It took us 1 year, 2 pairs of provisional teeth and several times of rebasations in order to reach the state of healthy, pink and stabile gingival tissues. After that we went on making the permanent construction. Due to all the information obtained in the planning and treatment phase, no corrections were made in the metallic probe as well as on the ceramic probe. One year ago the treatment of the maxilla was finished.
I continued the treatment on the mandibula, making a temporary removable prosthesis which I put on in the same appointment when I extracted teeth 45, 44, 42, 41, 31 and 32. The patient got very fast used to it but now he wants a better solution – either implant treatment, or a better kind of removable prosthesis. So the full mouth definitive solution is still up to evolve.
